Madagascar Tour Forms

Below are the items needed to participate in the Madagascar Tour arranged by due date. Click on the title of each item to download the corresponding form. For your convenience, there is also a checklist that you can download to keep track of the different steps.

All items can be turned in to cidp@andrews.edu or brought to our offices in Buller Hall 203.

Due by March 4, 2025:
Application This is your starting point and lets us know you'd like to join the tour. It also includes information about the forms process and due dates.
Passport Send us a scan/photo of your passport valid until at least November 7, 2025 - see attachment for more details
Dietary Restrictions/Preferences Let us know if you have any dietary needs/allergies/preferences (i.e. vegan, vegetarian, non-dairy)

This form along with the deposit officially secures your spot on the tour and allows us to start making travel bookings. Choose the one that is appropriate for you.

Make a deposit of $200 to "SSBS Tour - CIDP" in touchnet or at the SFS cashier - see attached for step by step instructions.
Non-students/guests/community participants can contact us for payment options.

Code of Conduct Agreement Agreement regarding conduct on the tour
Medical & Emergency Contact Fill out basic medical and emergency contact information so that we know how to help in case of an emergency
Assumption of Risk & Hold Harmless Risk Agreement
Proof of Insurance Send us a photo or scan of your insurance card (front & back) or downloaded proof of medical insurance when you send us your medical form
Financial Clearance - Undergrad
Financial Clearance - MSCID
Financial Clearance - MSW
Work with your financial advisor to complete this form.
Not required for non-students/guests/community participants.
Course Registration Students: Meet with your academic advisor to determine what 2 courses you should register for. Register for tour courses in registration central. We will send an email with course details when summer registration opens.

Minor Permission Form If any participants are under 18 years old, this parental/guardian permission form is required to join the tour
Travel Health & Safety Info We recomment you discuss antimalaria with your doctor. Antimalaria requires a prescription so we recommend seeing your doctor during break or making an appointment with the university medical center. See attachment for more details.
